Competition Details

Overview

The Memory Care Innovation Awards (“Awards”) takes place between May 1, 2025 and August 31, 2025. The Awards starts with a nomination process in which individuals or corporations will nominate a Memory Care Innovator for consideration as part of the program. WTWH Healthcare, a WTWH Media company, editors will evaluate each entry in its respective category to determine the Memory Care Innovation Awards Class of 2025.

Eligibility

All nominees must be employed by a for-profit, nonprofit, or governmental agency in the industries WTWH Healthcare, a WTWH Media company, and its publications covers and/or the businesses that support them. Each award entry fee must be paid prior to the entry deadline in order to be considered for eligibility for consideration and judging. Entry fees are non-refundable. Prior award winners should not be resubmitted for nomination to the program.

Nomination Process

Individuals, a corporation or governmental may nominate someone for the awards program via the online submission starting May 1, 2025 and complete the process prior to 11:59 PM Central Standard Time on August 31, 2025 for consideration. Upon submission, each individual will be entered into the Awards.

Competition Details

Below are the minimum requirements for the Memory Care Innovation Awards application process:

  • Complete the Entry Form with the following information:
    • Nominee’s name, organization, job title, email address, and phone number
    • Nominator’s name, organization, job title, email address, and phone number
    • Information on their work for recognition, including:
      • A maximum 500-word written statement outlining the nominee’s qualifications that address the judging criteria for the program (PDF or Word document)
      • Current resume or CV (PDF or Word document)
      • Nominee’s headshot (Hi-resolution JPG or PNG file)
      • Nominee’s headshot photo credit (if available)
      • Professional Letters of Recommendation
        • Submit a minimum of two (2) letters of recommendation (one (1) internal letter of recommendation and one (1) external letter of recommendation) or up to a maximum of four (4) letters of recommendation (at least one (1) has to be an external letter of recommendation) outlining why the nominee excels in memory care innovation based upon the criteria set forth for the competition. Nominators should include contact information (email, phone number) for a brief interview/vertification.

Each nominee, if not self-nominated, accepts these terms and conditions upon accepting the nomination and hereby agrees to the terms and conditions of the Awards. By entering the Awards, nominators and nominees agree to consent to WTWH Healthcare, a WTWH Media company, to use the personal information provided for the purposes of evaluating, judging and promoting as part of the agreement to participate in the program. Failure to claim or accept the nomination or meeting the eligibility requirements may result in disqualification or forfeiture of the participation in the Memory Care Innovation Awards Class of 2025.

Judging Criteria:

  • Leadership Qualities: Briefly describe the qualities of the nominee that provides examples of their innovative contributions to memory care and include specific examples that demonstrate their leadership acumen.
  • Innovation & Entrepreneurial Spirit: Describe the nominee’s ability to drive innovation in their organization, their initiative, and entrepreneurial spirit
  • Mission & Advocacy: Describe the positive contributions made by the nominee to the corporate and memory care industry advocacy/mission.
  • Ethics and Professionalism: Describe how the nominee has demonstrated exemplary ethics and professionalism to both the company and the memory care industry.
  • Clinical Excellence: Describe how the nominee has contributed to clinical excellence in the field of memory care, pushing for the highest standard of care and treatment provided to individuals experiencing memory-related disorders, such as Alzheimer’s disease and other forms of dementia, through any combination of medical expertise, compassionate care or a comprehensive understanding of the unique needs of individuals with memory impairments.

Entry Fees:

All entries are $500 USD.

Evaluation

All submissions will be evaluated by the WTWH Healthcare, a WTWH Media company, editorial staff. The editorial staff may select up to forty (40) members from the submissions. Selection of a candidate as a Memory Care Innovator is at the sole discretion of the editorial staff. The entries selected for the Memory Care Innovation Awards Class of 2025 for each category will be announced online on September 10, 2025.

Cancelation or Termination of Program

WTWH Healthcare, a WTWH Media company, reserves the right to cancel, suspend and/or modify the Awards, or any part of it, if any fraud, technical failures, virus, human error, tampering or any other factor impairs the integrity or proper functioning of the Awards, as determined by WTWH Healthcare, a WTWH Media company, in its sole discretion. In such event, WTWH Healthcare, a WTWH Media company, reserves the right to award the prizes from among the eligible entries or plays received up to the time of the impairment. WTWH Healthcare, a WTWH Media company, reserves the right, in its sole discretion, to disqualify any individual it finds to be tampering with the entry process or the operation of the or to be acting in violation of the Awards terms and conditions of this or any other promotion or disruptive manner. Any attempt by any person to deliberately undermine the legitimate operation of the Awards may be a violation of criminal and civil law, and, should such an attempt be made, WTWH Healthcare, a WTWH Media company, reserves the right to seek damages from any such person to the fullest extent permitted by law. WTWH Healthcare’s, a WTWH Media company, failure to enforce any term of these Official Rules shall not constitute a waiver of that provision.

Limitation of Liability

WTWH Healthcare, a WTWH Media company, is not responsible for interrupted or unavailable network server or other connections; for errors in communication protocols; failed telephone, computer or electronic transmissions; for jumbled, scrambled or misdirected entries or transmissions; for phone, electrical, network, technology or computer hardware or software or program malfunctions, failures or difficulties; or any other errors, omissions, interruptions, or deletions of any kind, whether human, mechanical or electronic; or for non-receipt of any entry regardless of the cause of it not being properly received. WTWH Healthcare, a WTWH Media company, is not responsible for unintelligible, late, lost or stolen entries; for incorrect or inaccurate entry information, whether caused by Internet users or by any of the equipment or programming associated with or utilized in the Awards; or for any technical, printing or human errors which may occur in the receipt, non-receipt or processing of any entries in the Awards. WTWH HEALTHCARE, A WTWH MEDIA COMPANY, MAKES NO WARRANTIES OF ANY KIND, EITHER EXPRESS OR IMPLIED, INCLUDING, BUT NOT LIMITED TO, IMPLIED WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE. SOME JURISDICTIONS MAY NOT ALLOW LIMITATIONS OF EXCLUSION OF LIABILITY FOR INCIDENTAL OR CONSEQUENTIAL DAMAGES OR EXCLUSION OF IMPLIED WARRANTIES. Privacy Policy

Any personally identifiable information collected during an applicant’s participation in the Awards will be collected by WTWH Healthcare, a WTWH Media company, and used in accordance with the WTWH Healthcare’s, a WTWH Media company, Privacy Policy. WTWH Healthcare, a WTWH Media company, will share the nominators’ name with the nominee, but not with the judges nor with the general public. Beyond the usage described in these terms and conditions, the information provided will be collected, stored and used in accordance with WTWH Healthcare’s, a WTWH Media company, privacy policy.

Publicity Rights

By submitting nomination, nominees (or persons entering on behalf of nominee) shall allow WTWH Healthcare, a WTWH Media company, and/or designees the perpetual right to use his/her entry, name, biographical information, photos, videos and/or likeness, and statements for promotion, trade, commercial, advertising and publicity purposes, at any time or times, in all media now known or hereafter discovered, worldwide, including but not limited to on the World Wide Web and Internet, without notice, review or approval and without additional compensation except where prohibited by law. A signed affidavit of eligibility, publicity release and release of liability may be required.

Governing Law

Issues concerning the construction, validity, interpretation and enforceability of these Official Rules shall be governed by the laws of Illinois.

Release

By participating, each nominee releases and agrees to indemnify and hold harmless WTWH Healthcare, a WTWH Media company, and Others from and against any and all costs, claims, damages, or any other injury, whether due to negligence or otherwise, to person(s) or property (including, without limitation, death or violation of any personal rights, such as violation of right of publicity/privacy, libel, or slander), due in whole or in part, directly or indirectly, to participation in the Awards.

General

WTWH Healthcare, a WTWH Media company, reserves the sole right to amend or alter the terms and conditions of the Awards at anytime. By entering the Awards, nominee agrees to abide and be bound by these Official Rules and the decisions of WTWH Healthcare, a WTWH Media company, which shall be final in all matters pertaining to this Awards program. All entries become the property of WTWH Healthcare, a WTWH Media company, and will not be returned. Void outside of the fifty (50) United States/District of Columbia and wherever prohibited or restricted by law. All federal, state and local laws and regulations apply.
